Required Forms
All student-athletes must be cleared online prior to participation in any sport or activity.
Please print and complete the Pre-Participation Physical Form (English or Spanish), the Student-Athlete Waiver Form, and the Physical Evaluation Form.
Please follow the steps below:
- Visit www.AthleticClearance.com.
- Watch the tutorial video.
- Register: Parents/Guardians should create an account using a valid email username and a password. You will be prompted to enter a code. Note: This step is essential for account activation. If you skip it, please contact us for assistance with activation.
- Log In using the email address you registered with.
- Select “New Clearance” to begin.
- Choose:
- The School Year in which the student plans to participate (e.g., Football in August 2024 would be the 2024-2025 School Year.)
- The School the student attends.
- The Sport.
- Complete all required fields for Student Information, Educational History, Medical History and Signature Forms. Physical forms (available below) may be download loaded but hard copies should still be submitted to the Front Office at your earliest convenience.

Once you see the Confirmation Message, you have completed the process. Print and return the signed consent form provided at the end of the process.

If you wish to register for additional sports or activities, you may select them below the Confirmation Message. Electronic signatures will be applied to these additional registrations.

All data will be electronically filed with the Athletic Department for review. You will receive an email notification once the student is cleared for participation.
